Christopher Rural Health Planning Corporation invites applications for a Nurse Practitioner or Physician Assistant to join our new West Frankfort primary care clinic in Illinois. Build your own patient panel in a newly developed facility with a four-day workweek and a salary range of $110,000–$135,000 annually based on experience.

Qualifications

• Graduate of an accredited NP or PA program.
• Current Illinois NP or PA licensure or eligibility to obtain licensure.
• Appropriate national certification.
• Strong clinical assessment, diagnostic, and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to practice independently within licensure scope while collaborating with other healthcare professionals.
• Commitment to providing compassionate, high-quality patient care.
• Primary care experience preferred; however, new graduates are encouraged to apply.
• Previous FQHC or community health experience is a plus but not required.

Responsibilities

• Perform physical examinations, health assessments, and preventive screenings.
• Diagnose and manage acute and chronic medical conditions.
• Develop individualized treatment plans and provide ongoing patient education.
• Order, perform, and interpret appropriate diagnostic testing.
• Prescribe medications and manage treatment in accordance with applicable licensure and practice requirements.
• Promote preventive care, wellness, and chronic disease management.
• Establish and maintain a patient panel while building strong relationships within the community.
• Collaborate with physicians, specialists, and other members of the healthcare team to coordinate patient care.
• Maintain accurate and timely documentation in the electronic health record.
• Participate in organizational quality improvement and patient care initiatives.
