Job summary

Evergreen by Park Capital Search is seeking a Family Nurse Practitioner for an on-site role in the Quad Cities. The clinic offers primary care for patients of all ages in a small, collegial team, with autonomy to manage your own patient panel.

The schedule is Monday–Friday, 8am–5pm, with no evenings, no weekends, and no on-call hours. This outpatient clinic emphasizes growth in family medicine, weight management, and wellness with physician collaboration and a robust referral network.

Qualifications

• Active Illinois APRN license with Family Nurse Practitioner certification (AANP or ANCC).
• 3+ years of experience as a Family Nurse Practitioner.
• DEA registration and Illinois controlled substance license or eligibility.

Responsibilities

• Conduct comprehensive health assessments including history and physical exams.
• Diagnose, treat, and manage acute and chronic conditions including preventive care.
• Review diagnostic results with patients and develop action plans.
