Cantex Continuing Care Network is seeking a Nurse Practitioner to provide patient-centered care for adults and elders in skilled nursing and long-term care settings. The NP will collaborate with physicians, RNs, case managers, and care teams to manage acute and chronic conditions and promote wellness.

The role includes clinical leadership, post-acute workflow, and performance targets, with a blend of onsite, facility-based, and administrative duties across Texas facilities.

Qualificações

• 2 years of NP experience in adult, geriatric, or family practice preferred.
• Ability to perform comprehensive adult/geriatric assessments and manage chronic disease.
• Identify risk factors, unmet needs, and gaps in care for senior populations.
• Proficient in medication reconciliation and managing complex patients.
• Graduate of an accredited NP program (Adult/Gerontology Primary Care preferred).
• Active RN and NP licensure in the state; board certified (AANP or ANCC).
• Current CPR certification.

Responsabilidades

• Perform comprehensive health risk assessments for assigned Members.
• Coordinate care with physicians, case managers, social workers, and facility staff.
• Identify quality improvement opportunities and develop individualized care plans.
• Review and interpret medical, surgical, and social histories.
• Perform screening procedures and interpret diagnostic data.
• Prescribe medications, order tests, and provide wellness counseling.
• Reconcile medications and ensure safe use.
• Refer Members to PCPs or disease management programs as needed.
• Maintain HIPAA and PHI compliance.
• Travel independently to assigned facilities as required.
• Serve as a clinical leader within the facility.
