UPLIFT HOMECARE, LLC is seeking a licensed Occupational Therapist to provide home-based OT services in Riverview, Brandon, and surrounding Hillsborough County. This role offers part-time and PRN opportunities with flexible scheduling and the potential for additional visits based on patient needs.

You will evaluate patients in their homes, develop individualized plans, and work with a supportive clinical team to deliver one-on-one care, with independence and autonomy in the field.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current and active Florida OT license.
• Minimum 1 year of home health experience.
• Current CPR certification.
• AHCA Level II Background Screening.
• Knowledge of state and federal healthcare laws and regulations.
• Strong clinical judgment and assessment skills.
• Excellent communication and interpersonal skills.

Responsabilités

• Evaluate and assess patients in homes; develop individualized treatment plans.
• Provide skilled OT services per patient plan of care.
• Improve ADLs, mobility, coordination, strength, safety and independence.
• Establish measurable therapy goals and monitor progress.
• Modify treatment plans based on patient response.
• Educate patients and caregivers on treatment plans, exercises, safety, and self-care.
• Identify fall risks and recommend safety strategies and equipment when indicated.
• Communicate progress with physicians and care team; maintain accurate documentation.
• Coordinate care with nurses, PTs, STs, CNAs, physicians and other professionals.

What You'll Do

As a Home Health Occupational Therapist, you will:

• Evaluate and assess patients in their homes to determine functional needs and develop individualized treatment plans
• Provide skilled occupational therapy services according to the patient's plan of care
• Implement therapeutic interventions to improve activities of daily living, functional mobility, coordination, strength, safety, and independence
• Establish measurable therapy goals and monitor patient progress
• Modify treatment plans based on patient response and clinical findings
• Educate patients and caregivers regarding treatment plans, therapeutic exercises, adaptive techniques, home safety, fall prevention, and self-care
• Identify fall risks and recommend appropriate safety strategies and adaptive equipment when indicated
• Communicate patient progress and changes in condition with physicians and members of the interdisciplinary care team
• Maintain accurate, timely, and compliant clinical documentation
• Coordinate care with nurses, PTs, STs, CNAs, physicians, and other healthcare professionals

Requirements

• Graduate of an accredited Occupational Therapy program
• Current and active Florida Occupational Therapist (OT) license
• Minimum 1 year of home health experience required
• Current CPR certification
• AHCA Level II Background Screening
• Knowledge of applicable state and federal healthcare laws and regulations
• Strong clinical judgment and assessment skills
• Excellent communication and interpersonal skills
• Compassionate, dependable, resourceful, and patient-focused
• Ability to work independently and effectively manage time in a home health environment
• Reliable transportation
• Valid driver's license and appropriate auto insurance

The bonus will be paid through regular payroll following completion of the 90-day eligibility period and is subject to applicable taxes and company eligibility requirements.
