Job summary

Optum, in Sedona, seeks an Afterhours Registered Nurse for hospice care, including night and weekend shifts. You will deliver comprehensive hospice services, triage by phone, perform admissions and visits, and coordinate with families to meet physical and psychosocial needs.

Requirements include an active RN license in the state, CPR certification, 1+ year of clinical experience, and a valid driver's license with reliable transportation. A $7,500 sign-on bonus is offered for external candidates.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current CPR Certification.
• 1+ years of clinical experience.
• Current driver's license and access to a dependable vehicle.

Responsibilities

• Triage patient needs by phone and arrange in-person visits as needed.
• Respond to after-hours crisis calls with urgency.
• Make initial nursing evaluation within 48 hours of referral if assigned.
• Conduct afterhours visits including admissions, death, recertification, and routine visits.
• Assess and reassess patient/family needs every 15 days at minimum.
• Document problems, goals, interventions, and patient/family responses.
