Job summary

TriHealth is seeking a Night Triage Registered Nurse (RN) to provide expert telephone triage, clinical assessment, and care coordination for hospice patients and families during overnight hours.

The role involves responding to after-hours calls, assessing symptoms, delivering interventions and education, coordinating visits, and ensuring 24/7 access to support. Travel to patient locations is required as part of hospice care delivery.

Qualifications

• Requires nursing licensure and clinical judgment from nursing education.
• BSN completion within 5 years of hire is preferred.
• Experience in hospice, home care, or triage is beneficial.

Responsibilities

• Perform initial and ongoing patient and family assessments.
• Document assessments per TriHealth guidelines.
• Communicate assessment findings to health care providers.
• Provide education and care coordination for hospice patients and families.
• Coordinate visits and ensure timely access to support 24/7.
