Job summary

Responsibilities

Conduct comprehensive assessments of patients across physiological, psychological, developmental, sociocultural, spiritual and lifestyle factors.
Develop and implement individualized care plans and reevaluate as needed.
Administer medications and treatments safely per physician orders and protocols.
Educate patients and families about conditions, treatments and self-care to promote understanding.
Coordinate with medical staff and departments to optimize patient care and teamwork.
Maintain accurate documentation of patient information, care activities, interventions and outcomes.
