Signature HealthCARE at Jefferson Manor Rehab & Wellness Center in Shepherdsville, KY is seeking an experienced RN for the night shift (6:30 PM–7:00 AM) to deliver high-quality nursing care in a 100-bed facility. The ideal candidate holds an active RN license in Kentucky, with 1+ year of clinical nursing experience; LTC experience preferred and supervisory exposure.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Strong verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
