Job summary

Great River Health in West Burlington, IA is seeking a Transfer Center Coordinator RN to orchestrate patient transfers across facilities. You will serve as the main communication hub between referring facilities, physicians, bed management, and ancillary departments to ensure safe, timely placement based on clinical needs and bed availability.

The role requires an Iowa RN license and a background in clinical assessment and coordination, with experience in high-stress, time-sensitive

Qualifications

• Registered Nurse license in Iowa (IBON) required upon hire.
• AA degree required; BA/BS in Nursing or Healthcare Management preferred.
• 1–3 years in healthcare operations preferred.
• 4–6 years in patient transfer coordination preferred.
• 4–6 years in patient care preferred.
• Strong communication and coordination across disciplines.

Responsibilities

• Coordinate communication between transferring and receiving medical staff.
• Arrange transportation and accommodations for patient transfer.
• Ensure compliance with transfer regulations.
• Maintain detailed records of transfer activities for quality assurance.
• Monitor and evaluate transportation logistics and efficiency.
• Provide training to new staff on transfer protocols.
