Qualifications

• Active RN license in the state of practice.
• 1-3 years of experience in a long-term care facility preferred.
• Strong communication and leadership abilities are required to supervise care delivery.

Responsibilities

• Provides leadership and day-to-day management. Guides staff in meeting needs of residents.
• Coordinates admissions, discharges and transfers and participates in care planning.
• Ensures completion of Resident Assessment Instrument (RAI) including MDS, RAPs and Care Plans.
• Maintains safe, infection-control compliant environment and proper medication administration.
• Assists with data collection and quality improvement activities, contributes to QAPI plans.
• Communicates care plans and coordinates with the health care team and residents' families.
