Job summary

Avamere Rehab of Junction City in Junction City, OR is seeking a Registered Nurse (RN) to deliver comprehensive patient care on a 12-hour night shift (6p-6a) at the Avamere Rehab facility. The RN will assess residents, administer medications, monitor health, document care, and supervise LPNs/CNAs, collaborating with the interdisciplinary team to ensure high-quality outcomes and safety.

Experience in long-term care and EMR documentation is preferred; must hold an active RN license in Oregon and

Qualifications

Must possess a nursing degree from an accredited college or university.
Must possess a current, active RN license in Oregon (BSN preferred).
Must maintain active CPR/BLS during employment.
Experience in long-term care, geriatric rehab, sub-acute, or critical care setting preferred.
Experience with electronic medical records documentation.

Responsibilities

Assess residents and develop individualized care plans with the interdisciplinary team.
Administer medications and monitor residents’ responses for safety.
Monitor vital signs and health status; document interventions.
Supervise and train LPNs and CNAs; provide orientation for new staff.
Maintain accurate care documentation and communicate with residents, families, and healthcare professionals.
