Qualifications

• 1 year of experience in Skilled Nursing, Long-Term Care, or rehabilitation settings.

Responsabilités

• Provide direct patient care, including assessments and care planning.
• Collaborate with interdisciplinary teams to support recovery.
• Educate patients and families on post-discharge care and rehab plans.
