Job summary

LiveWell Homecare Agency, LLC seeks an RN to provide patient- and family-centered care in a Lansdale location. You will apply the nursing process to address physical, psychological, emotional, and social needs and develop individualized care plans with patients and families.

You will collaborate with healthcare teams, deliver tailored education, and participate in safety and quality initiatives while benefiting from comprehensive benefits and growth opportunities.

Qualifications

• Accredited nursing diploma and active RN licensure in the practice state.
• BSN preferred within three years.
• Strong communication, assessment, education and documentation skills.
• BLS certification required.

Responsibilities

• Deliver patient- and family-centered care using the nursing process.
• Develop individualized care plans in collaboration with patients and families.
• Communicate effectively with patients, families, and healthcare teams.
• Provide education tailored to patient and family needs.
• Use health information technology for communication, documentation, and quality improvement.
• Collaborate within inter-professional teams and support shared decision-making.
• Participate in safety and quality initiatives and pursue ongoing professional development.
