Qualifications

Responsibilities

• Promotes patient and family centered care in a healing environment.
• Educates patients and their families on illness, post treatment home care needs and medication administration.
• Participates in safety, quality and regulatory compliance; participates in professional development.
• Develops, implements, and documents individual plans of care with defined goals in collaboration with the interprofessional team.
• Assesses and communicates patient needs and preferences across the continuum of care.
• This is a safety-sensitive position requiring alertness and safe practices.
