Qualifications

• RN with appropriate licensure and nursing education as listed.
• Experience preferred to support clinical needs and safety.

Responsibilities

• Provides direct patient care and assessment within standards of nursing care.
• Develops, implements, and documents plans of care with the interprofessional team.
• Educates patients and families on illness management and home care needs.
• Promotes patient safety, quality and regulatory compliance, and professional development.
• Coordinates care across the continuum and communicates plans effectively.
