Qualifications

• Nursing diploma/Associate's degree required
• RN license required
• Ability to provide direct patient care

Responsibilities

• Provides direct patient care including assessment, planning, implementation and evaluation in nursing standards.
• Educates patients and families on illness management and home care needs.
• Promotes patient safety and quality by following regulations and participating in ongoing professional development.
• Develops and documents care plans with interdisciplinary team and ensures continuity of care.
