Job summary

MercyOne Northeast Iowa is seeking a part-time nurse on a 24/7 nurse advice and triage team. The role is 0.6 FTE with a 12-hour night shift (7 pm–7 am) and an every third weekend rotation, onsite at MercyOne facilities. A 3-year nursing experience requirement is noted, with an Associate's degree required and Bachelor’s preferred.

• Associate's degree in Nursing required
• Bachelor's degree preferred
• Minimum 3 years of nursing experience

Responsibilities

• Receives, reviews and acts on hospital discharge reports for patients who have not scheduled follow-up visits
• Performs telephone triage and initial data assessment to guide care
• Educates patients and families to meet identified needs
• Identifies short-term goals and develops an outcome-based care plan
• Collaborates with Clinic Medical Home Team and adheres to safety and infection control policies
