Responsibilities

• Perform comprehensive patient health assessments and interpret data.
• Develop individualized plans of care based on assessments.
• Provide patient- and family-centered care across the continuum.
• Advocate for patients' rights and coordinate care with families.
• Document care activities and coordinate with the healthcare team.
• Contribute to performance improvement and education efforts.
• May perform additional duties as assigned.

BSN from an accredited school of nursing
