Qualifications

• Must possess a current RN license in the state of practice.
• BLS certification is required.
• Experience in acute care is preferred.

Responsibilities

• Assess patient needs and discharge planning requirements.
• Formulate and revise the plan of care in collaboration with patients and families.
• Deliver direct patient care and coordinate with the healthcare team.
• Document nursing assessments, plans, interventions and patient responses.
• Provide mentorship and support to other nursing staff as needed.
