Job summary

Qualifications

Responsibilities

• Demonstrates safety and continuity of care using documentation and handoff tools.
• Creates plan of care using the nursing process: assessment, diagnoses, interventions, and evaluation of outcomes.
• Provides education based on patient learning needs and cultural sensitivity.
• Assesses patient data to identify age-specific requirements and plan individualized care.
• Performs patient assessments and accurate documentation.
• Demonstrates evidence-based nursing care and participates in patient-centered care models.
