• Plans, implements, and evaluates patient care plans and communicates patient condition.
• Collaborates across disciplines for transfer, discharge, referrals, and psychosocial needs.
• Provides patient and family education tailored to age, culture, condition, and circumstances.
• Effectively delegates patient care to ancillary personnel and may precept students/new hires.
