• Develop and implement individualized care plans with measurable goals.
• Prioritize and complete follow-up assessments and modify plans as needed.
• Communicate effectively with patients, families, and the interdisciplinary team.
• Engage in hand-offs, education, and charting per policy.
• Participate in quality improvement initiatives and regulatory compliance.
