Qualifications

Responsibilities

• Provide prescribed medical treatment and personal care services to residents in Long-Term Care.
• Assess, plan, intervene, and evaluate resident care using the nursing process.
• Perform admission assessments and home med reconciliation.
• Document and prioritize resident data across physical, psychosocial, spiritual, educational needs.
• Communicate with residents, families, physicians, and staff to coordinate care.
