Job summary

45 Fayette County Hospital is seeking a Licensed RN for Long-Term Care to assess, plan, implement, and evaluate nursing care for residents in a skilled nursing setting. You will oversee CNAs and support staff, educate families, and ensure continuity of care across shifts in a 6:00pm–6:00am schedule with weekend/holiday rotation.

Responsibilities include pain management, medication administration, accurate documentation, and collaborative discharge planning.

Qualifications

• ADN required.
• BLS or CPR certification required within 90 days of hire.

Responsibilities

• Assess and reassess resident pain and needs.
• Administer medications and treatments per protocol.
• Review physician orders and take appropriate actions.
• Document care, assessments, and changes in condition.
• Coordinate discharge planning with the care team.
• Educate residents and families about plan of care and pain management.
• Respond to call lights promptly and ensure continuity of care across shifts.
