Job summary

Fayette County Hospital seeks an LPN LTC to provide general care, assess residents, document procedures, educate caregivers, and support safe, quality care delivery. The role follows 6:00pm–6:00am shifts with weekend/holiday rotation.

Pay starts at $23.87/hour plus shift and weekend differentials; compensation ranges from $23.87 to $37.00/hour based on experience. Responsibilities include pain management, medication administration, and care coordination.

Qualifications

• High School required.
• BLS or CPR certification within 90 days of hire.
• Graduate of Accredited LPN Program.
• LPN - Licensed Practical Nurse.
• Illinois Department of Financial and Professional Regulation.

Responsibilities

• Assesses and reassesses pain and educates resident and family on pain management.
• Administers medications and treatments per protocol and reviews physician orders.
• Assesses residents on admission and quarterly; documents in chart.
• Coordinates discharge planning with the care team.
• Documents changes promptly; ensures notifications are made.
