Qualifications

• BSN preferred; required as of 4/27/2025 update.

Responsibilities

• Assess patients across physical, social, and psychological domains and gather information from the interdisciplinary team.
• Plan individualized care with patient/family involvement, including teaching and referrals.
• Implement care by directing the interdisciplinary team, prioritizing, and managing time and resources.
• Evaluate patient response and adjust the care plan as needed to meet goals and ensure satisfaction.
• Demonstrate professionalism, comply with licensure standards, and maintain accurate documentation.
