Responsibilities include assessing resident needs, developing and implementing individualized care plans with the care team, and coordinating discharge planning.

Qualifications

• Identifies patient care needs and input for discharge planning and follow-up.
• Develops individualized care plans with the interdisciplinary team.
• Implements plans of care focusing on resident-centered outcomes.
• Collaborates with physicians and other health care professionals.
• Delivers optimum resident care and comfort using current nursing practices.
• Must hold an active RN license in the state of practice.
• CPR certification required within 90 days of hire.
• No findings of abuse, neglect, or misappropriation in the registry.

Responsibilities
