Job summary

Qualifications

Responsibilities

• Assess: perform comprehensive assessment of physical, social, and psychological status and gather information from the care team; patient, family, physician, and others.
• Plan: collaboratively develop an individualized plan of care with active involvement from patient, family, team, and physician.
• Implement: direct the care team to deliver care per plan, manage time, supplies, and resources.
• Evaluate: review outcomes and revise the plan of care as needed.
• Professionalism: uphold ethical standards, document accurately, and comply with licensure and regulatory requirements.
