Responsibilities

• Collects data relevant to patient health and analyzes assessment data.
• Develops care plans and coordinates delivery with the healthcare team.
• Participates in discharge planning to ensure continuity of care.
• Delegates duties and oversees nursing staff as assigned.
• Performs other job-related duties as assigned.
