• Coordinate and deliver high-quality, patient-centered care per department standards.
• Collaborate with providers and care team to create and monitor treatment plans.
• Document care thoroughly in the medical record and respond to patient needs.
• Act as advocate for patients/families to support safe, effective care.
• Support transition of care and interface with community resources.
