Qualifications

• Basic Life Support Certification (BLS) for healthcare providers.

Responsibilities

• Assess physical, social, and psychological status; gather information from the care team.
• Develop an individualized care plan with patient/family involvement and education.
• Coordinate an interdisciplinary care plan and prepare for follow-up needs.
• Direct and delegate care, manage time and resources effectively.
• Evaluate outcomes and revise the care plan as needed.
• Maintain documentation and uphold professionalism and licensure compliance.
