Woburn Rehabilitation and Nursing Center is seeking a compassionate Registered Nurse (RN) to join its night shift team. The role includes direct patient care, medication administration, assessments, and collaboration with a multidisciplinary team.

Qualifications

• Current, active Massachusetts Registered Nurse (RN) license in good standing.
• Minimum of 2 years of RN experience in a skilled nursing facility (SNF).
• Strong clinical assessment, critical-thinking, and decision-making skills.
• Excellent communication and teamwork abilities.
• Ability to prioritize responsibilities in a fast-paced environment.
• Compassionate, reliable, and committed to high-quality resident care.

Responsibilities

• Provide direct nursing care to residents, including medication administration, vital signs, and ongoing assessments.
• Monitor residents' conditions and promptly report changes to the healthcare team.
• Develop, implement, and update individualized care plans with the interdisciplinary team.
• Communicate effectively with residents, families, physicians, and care team.
• Maintain accurate documentation of nursing care and observations.
• Support a safe, comfortable nighttime environment and respond to residents' needs.
