Responsibilities

• Provide direct nursing care to long-term care residents per established clinical standards.
• Implement and monitor individualized care plans, document and communicate changes in condition.
• Coordinate with physicians and interdisciplinary teams to carry out orders and ensure seamless care.
• Promote resident safety and support effective discharge planning.
