Zusammenfassung

YoloCares is seeking a Registered Nurse (RN) for the Night Shift to provide after-hours nursing assessments, triage, and in-person care to hospice patients and families in Yolo and surrounding counties. You will report to the Clinical Manager and participate in coordinated interdisciplinary care to ensure comfort and symptom management.

Responsibilities include telephone triage, home visits when needed, and documentation in the EMR, while collaborating with physicians, nurses, and social workers

Qualifikationen

• Current California RN license required.
• Current CPR certification required.
• Hospice experience preferred (1 year).
• Experience with EMR systems (Allscripts/FireNote preferred).
• Strong communication and teamwork with interdisciplinary staff.
• Ability to travel within service area.

• Perform after-hours triage and nursing assessments.
• Conduct home visits as clinically indicated.
• Provide telephone guidance, education, and symptom management.
• Document patient status and changes in condition in EMR.
• Coordinate care with day/recertification teams and physicians.
