The Medical University of South Carolina (MUSC) Community Physicians team is seeking a QMHP / Social Worker to support discharge planning and care coordination for patients across South Carolina. Under clinical guidance, you will assess patients and caregivers, identify discharge risks, and bridge the gap to outpatient care with the care team.

Qualified candidates hold an MSW and SC licensure; certification in case management is preferred.

Qualifications

• Master's degree in social work or social welfare program accredited by CSWE (Council on Social Work Education).
• MSW licensure by SC Board of Social Work Examiners.
• Certification in case management preferred.

Responsibilities

• Assist with discharge coordination to ensure a smooth transition in collaboration with the health care team.
• Complete patient and caregiver assessments during hospitalization and follow-up.
• Identify post-discharge risk factors and apply interventions to reduce adverse events.
