Medical University of South Carolina (MUSC) Community Physicians in Lexington County Detention Center is seeking a QMHP / Social Worker to coordinate discharge planning and ensure a successful transition in collaboration with the health care team.

The MSW-credentialed professional assesses patients and caregivers, identifies discharge risks, and connects patients to outpatient providers to improve outcomes.

Qualifications

• Master's degree in a social work or social welfare program accredited by the CSWE.
• Licensure as MSW by the SC Board of Social Work Examiners.
• Certification in case management preferred.

Responsibilities

• Coordinate the discharge process with the health care team to achieve desired patient outcomes.
• Complete patient and caregiver assessments during hospitalization and follow-up.
• Identify post-discharge risk factors and apply interventions to reduce risk of adverse events after discharge.
• Bridge discharge planning to outpatient providers and address questions with patient and caregiver.

Job Description

Assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The QMHP / Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.

Education:

Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
