Job summary

MUSC Health seeks a QMHP / Social Worker (MSW) to support discharge planning and care coordination for patients at the Lexington County Detention Center. You will assess patients and caregivers during hospitalization and follow-up, identify discharge risks, and collaborate with the health care team to ensure safe transitions.

The role bridges discharge to outpatient providers, with emphasis on addressing patient and caregiver questions and ensuring effective continuity of care.

Qualifications

• Master's degree in a social work or social welfare program accredited by CSWE.
• MSW licensure by the South Carolina Board of Social Work Examiners.
• Certification in case management preferred.

Responsibilities

• Assist with coordination of the discharge process to ensure a smooth transition in collaboration with the healthcare team.
• Conduct assessments on patients and caregivers during hospitalization and outpatient follow-up.
• Identify post-discharge risk factors and apply interventions to reduce risk of adverse events.
• Bridge discharge to outpatient providers, addressing issues and questions with patients and caregivers.
