MUSC Community Physicians (MCP) is seeking a Social Worker to coordinate discharge planning and collaborate with the health care team to ensure a smooth transition for patients. The role requires completing assessments for patients and caregivers during hospitalization and post-discharge follow-up.

The MSW credential is required by the South Carolina Board of Social Work Examiners, with certification in case management preferred.

Qualifications

• Master’s degree in social work required.
• MSW licensure by SC Board of Social Work Examiners required.
• Certification in case management preferred.

Responsibilities

• Coordinate the discharge process with the health care team to ensure a successful transition.
• Complete patient and caregiver assessments during hospitalization and follow-up.
• Identify discharge-related risk factors and apply interventions to reduce post-discharge risk.
• Bridge discharge planning to outpatient providers with patient and caregiver.
