Job summary

Qualifications

• BLS for healthcare providers is required.

Responsibilities

• Assess patient physical, social, and psychological status and information from interdisciplinary team.
• Collaborate to develop individualized plan of care and involve patient and family in planning.
• Direct and coordinate care delivery, prioritize tasks, and manage time and resources.
• Evaluate patient responses and revise the care plan as needed.
• Promote professionalism, comply with regulatory standards, and maintain accurate documentation.
