Responsibilities

• Assist nursing staff with delegated basic patient care and maintain a clean, safe environment
• Record vital signs and notify RN/LPN of changes
• Support patients with meals, bathing, grooming, and hygiene
• Assist with mobility and use of walkers, canes, or wheelchairs
• Collect samples and monitor glucose using finger sticks
• Communicate patient information to the care team promptly
• Adhere to infection control protocols and PPE guidelines
• Participate in performance improvement and risk management activities
• Maintain regular attendance and comply with policies
