Qualifications

Responsibilities

• Assists nursing staff in delivering care and maintains a clean, safe environment.
• Records vital signs, intake/output, and reports findings to RN/LPN/LVN.
• Supports patients with meals, bathing, grooming, and mobility needs.
• Maintains infection control practices and proper PPE usage.
• Communicates patient information effectively to the care team.
• Participates in performance improvement and risk management activities.
• Maintains regular attendance and may provide continuous observation as required.
• Performs other duties as assigned.
