Job summary

Trinity Health MercyOne in Des Moines is seeking a Patient Logistics Access Specialist for the night shift (11:00 PM–7:00 AM). You will be the central contact for incoming transfer and admission requests, collecting essential patient and referral information to initiate transfers.

You will route calls to clinicians and coordinate non-clinical transport logistics across facilities and EMS partners. The role emphasizes accurate documentation, regulatory awareness, and maintaining patient safety

Qualifications

• Health care experience required.
• High school diploma or GED required.
• Associate’s Degree or equal experience preferred.
• Must be comfortable operating in a collaborative, shared leadership environment.

Responsibilities

• Serves as the first point of contact for incoming transfer, admission, and consultation requests
• Collects and verifies demographic, referral, and operational information to initiate the transfer workflow
• Manages multi-line phone activity to support timely patient transitions
• Documents call details in electronic health records and transfer systems accurately
• Routes calls to appropriate clinicians or operational team members per protocol
• Supports EMTALA-sensitive processes by ensuring complete intake documentation and escalation when red flags arise
• Coordinates non-clinical transport logistics and communicates status changes to EMS and receiving units
