Qualifications

• BSN from an accredited nursing program.
• Current NY State RN license.
• Minimum 2 years of supervision in assisted living, nursing homes or similar.
• Experience with dementia/Alzheimer’s care is required.
• Experience with assessments, care planning and reassessments.

Responsibilities

• Oversee health-related services for ALR/ALP.
• Assess applicants to determine care appropriateness.
• Develop and implement ISP based on assessments.
• Conduct reassessments and adjust care plans as needed.
• Document resident care and changes in status.
• Supervise program nurses and home health aides.
