The role emphasizes coordinating healthcare and wellness programs for residents in Assisted Living and Adult Care facilities, with focus on assessments, ISP development, and staff supervision.

The AL-Nurse Manager collaborates with nursing professionals to improve patient outcomes, reduce hospitalizations, and enhance residents’ functional levels.

Qualifications

• Graduate of an accredited school of Nursing (BSN).
• Current Registered Nurse New York State License.
• Experience in staff supervision and medication oversight in Assisted Living, Nursing Homes, or similar environments.
• Strong management and supervisory skills, with experience in assessments, care planning, and reassessments.
• Experience with Dementia and/or Alzheimer’s patients is a must.
• Minimum of 2 years of experience providing services to older adults in settings such as home care, residential care, adult care facilities, or rehabilitation.

Responsibilities

• Oversee healthcare and wellness programs for residents in Assisted Living or Adult Care facilities.
• Coordinate clinical assessments, care plans, and service delivery.
• Supervise program nurses and home health aides.
• Assist in development and implementation of ISP and assessment schedules.
• Document resident care and changes in status.
