Qualifications

• Current LPN or RN license in the state.
• At least one year of LPN/RN experience in a long-term care setting preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Coordinate with healthcare team to assess, plan, implement, and evaluate patient care.
• Prepare and administer medications (oral, subcutaneous, IV) and record as prescribed.
• Monitor patients, read vital signs, and adjust care as needed.
• Assist with discharge planning and patient education.
