Qualifications

Responsibilities

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
• Develop and implement individualized care plans for each patient, in collaboration with the healthcare team.
• Monitor and report any changes in patient condition to the Charge Nurse or Physicians.
• Assist with resident transfers, ambulations, and bedside care.
• Maintain accurate and detailed records of patient care and treatment.
• Collaborate with other healthcare professionals to ensure continuity of care.
