Job summary

Decatur Wellness & Rehabilitation Center in Decatur, TN is seeking a compassionate Night Shift LPN (6 PM–6 AM) to lead nursing care for residents. You will supervise CNAs, manage medications per physician orders, and participate in admissions, discharges, and transfers, while updating care plans and maintaining confidentiality.

Current Tennessee LPN license and CPR/BLS certification are required; a sign-on bonus of $7,500 is offered along with competitive benefits and a supportive team

Qualifications

• LPN license for Tennessee must be current and unrestricted.
• CPR/BLS certification required.
• Experience directing CNAs and coordinating resident care is preferred.

Responsibilities

• Direct the daily activities of CNAs and ensure policy adherence.
• Participate in admissions, discharges and transfers of residents.
• Assess resident conditions and administer medications per orders.
• Initiate, review and update care plans.
• Manage nursing care and delegate shift tasks to nurses.
• Supervise nurse aides and monitor resident status.

LPN Position Summary

As a LPN at Decatur Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Manage the nursing care for all residents and delegate shift tasks to nurses.
• Supervise nurse aides as they complete their assignments.
• Observe residents’ conditions, oversee, and respond as necessary.
• Inform physicians of any changes in resident conditions.

• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

LPN Requirements

• Current unrestricted LPN License for the state of Tennessee
• Current CPR/BLS Certification
