Job summary

Qualifications

• Bachelor's degree in human services or related field from an accredited program.
• Knowledge of crisis therapy and intervention techniques.
• 2+ years working with individuals with chronic mental illness or ED experience.

Responsibilities

• Receives transferred mental or behavioral health calls from 911 dispatch.
• Performs triage screening and psychosocial assessment; documents in client record.
• Conducts ongoing data collection focusing on environment, self-care and barriers.
• Provides response and interim plan for identified needs.
• Forms individualized safety plan and connects with support system.
• Engages clients for ongoing assessment, support, and motivational interviewing.
• Attends team meetings and contributes to multidisciplinary treatment goals.
• Assists in program development and client activities.
• Obtains and provides transportation for clients as needed.
• Maintains accurate documentation per department standards.
• Maintains knowledge of community resources and access points.
• Provides referrals to Broadlawns and community resources.
• Back-up to other Community Crisis positions as needed.
• Performs other duties as assigned.
