Job summary

Qualifications

• Must hold NC LPN license.
• Diploma in Practical Nursing required.
• High School Diploma or equivalent required.
• 1 year acute care experience preferred.

Responsibilities

• Conduct comprehensive assessments to develop individualized care plans.
• Performs treatments following established protocols and practices.
• Monitors and evaluates patient progress; documents findings in EMR and notifies the care team of changes.
• Educates patients, families, and caregivers on care techniques, prevention strategies, and signs of complications.
• Collaborates with physicians, registered nurses, and other healthcare professionals to coordinate care and optimize outcomes.
• Maintains accurate and up-to-date documentation of assessments and outcomes in the EMR.
• Performs other duties as assigned.
