Job summary

Advocate Health’s Carlyle Place in Macon, GA seeks an LPN to provide nursing care across Skilled Nursing, Assisted Living, and Memory Care settings, under supervision of a RN. The role focuses on comfort, wellness and quality outcomes.

You’ll participate in assessments, administer medications, and contribute to individualized care plans while delivering resident-centered, compassionate care in a welcoming senior living community.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted Georgia LPN license (or compact license eligible for practice in Georgia).
• Current CPR certification or ability to obtain within 60 days of hire.
• Basic Life Support (BLS) certification required per policy.
• Skilled nursing, long-term care, assisted living, or memory care experience preferred.

Responsibilities

• Provide compassionate nursing care to residents in Skilled Nursing, Assisted Living, and Memory Care settings.
• Monitor resident conditions and report significant changes to the healthcare team.
• Assist with assessments, care planning, treatments, and procedures.
• Administer medications and treatments within scope of practice.
• Educate residents and families regarding care plans and treatments.
• Document assessments, interventions, and resident responses accurately and timely.
• Collaborate with interdisciplinary team members to support positive resident outcomes.
• Ensure care is delivered safely, efficiently, and in accordance with regulatory requirements.
